This form must be signed by the student. It must also be signed by the parent or guardian of any student who has not yet reached 18. 

Read carefully: This form involves a waiver and release of legal rights. 

I understand that during TONGWOOD activities, students will have opportunities to participate in various field trips, such as visits to art museums, outdoor sketching sessions, park visits, and other related activities. I acknowledge that such field trips are an integral part of the TONGWOOD program, and I grant my permission for the student to participate in all such trips. 

In consideration of the student having the opportunity to participate in these trips, I (on behalf of myself, my child, and our respective heirs) hereby release TONGWOOD and all of its affiliates, employees, agents, trustees, and any other person or entity acting on its behalf from any liability, loss, damage, or claim of any kind that in any way arises from or relates to the student’s participation in any such field trip, including but not limited to any claim arising from the alleged negligence of TONGWOOD or any person acting on its behalf. 

I further agree to release the owners and operators of any properties or activities visited during such field trips, as well as any contractors or subcontractors working on the property or activity, from any liability, loss, damage, or claim of any kind that in any way arises from or relates to the student’s participation in any such field trip, including but not limited to any claim arising from the alleged negligence of any such owner, operator, contractor, or subcontractor or any person acting on their behalf.
