Waiver

Children under 18 are REQUIRED to have their waivers signed and initialed by a parent or legal guardian (preferably the one accompanying them to TONGWOOD) prior to participating in classes, workshops or drop-in activities at TONGWOOD.  

 

Our waivers are completely confidential.  None of your personal information will be given away or sold.

Release and Indemnification Agreement

I represent that I am the parent or legal guardian of the child I am enrolling, who is participating in a semester, camp day, program or other event (hereinafter the “Program”) by or with TONGWOOD LLC (hereinafter “TONGWOOD”). I hereby give my authorization and approval for my child’s participation in any and all of the activities of the Program. I further acknowledge and agree that participation in the Program.

 

I further acknowledge that the above is not inclusive of all possible risks associated with the Program and that the above in no way limits the extent or reach of this release.

 

By enrolling my child in the Program, and in consideration for the opportunity to participate in the Program, to the fullest extent permitted by law, I hereby release, indemnify and hold harmless on behalf of myself, my child, our respective heirs, representatives, executors, administrators, and assigns (each an “Indemnitor”) TONGWOOD, its officers, agents and employees from any and all causes of action, claims, or demands of any nature whatsoever, including but not limited to a claim of negligence, which I and any Indemnitor may now have or have in the future against TONGWOOD arising out of or in any way related to my child’s enrollment, or inability to enroll, in the Program.

 

Checking the appropriate box during online registration, I acknowledge that I have read this Release and Indemnification and understand that it includes an assumption of the risk and a release of liability. I understand that TONGWOOD is relying on this waiver in allowing my child to participate in the Program. I hereby certify that my child has no limitations that with or without a reasonable accommodation would preclude his/her safe participation in the Program. To ensure the safety of your child and the safety of others, please describe in the register space all attributes of your child that may require a reasonable accommodation, including, without limitation, any need for medication, all allergies, and any other physical or mental attribute that may need reasonable accommodation.
